Objective: To identify the level of therapeutic adherence in older adults diagnosed with hypertension. Methodology: A cross-sectional, descriptive, quantitative study was carried out in 129 older adults with a diagnosis of systemic arterial hypertension. The information was obtained through a sociodemographic data questionnaire and through the Therapeutic Adherence Scale instrument. Results or Findings: Female sex predominated (69.0%), (79.1%) were in regular adherence, while only (20.9%) had good adherence. Conclusions: Important results were obtained from this study, which can have a great impact on the development of nursing knowledge in the approach to this population. Introducción. A nivel mundial se estima que existen 1130 millones de personas con hipertensión arterial, la mayoría de ellas vive en países de ingresos bajos y medianos, es una de las principales causas de muertes prematuras en el mundo, de acuerdo a una estadística de la Organización Mundial de la Salud (OMS) en 2015, arrojó que uno de cada cuatro hombres padece hipertensión arterial, mientras que, en mujeres, una de cada cinco. Objetivo. Identificar el nivel de adherencia terapéutica en adultos mayores con diagnóstico de hipertensión arterial. Metodología. Se realizó un estudio cuantitativo tipo descriptivo, transversal, en 129 Adultos Mayores con diagnóstico de hipertensión arterial. La información se obtuvo a través de una cédula de datos sociodemográficos y a través del instrumento Escala de Adherencia Terapéutica. Resultados. Predomino el sexo femenino (69.0%), el 79.1% se encuentra en regular adherencia, mientras que solo el 20.9% tiene una buena adherencia, las mujeres presentaron una mayor adherencia terapéutica en comparación con los hombres (p<.05). Conclusión. El abordaje de la adherencia terapéutica en adultos mayores evidencia la importancia, así como la necesidad de brindar atención personalizada en cuestión de autocuidado con la finalidad de mejorar la adherencia terapéutica en esta población vulnerable.
