Encefalopatís diagnosing Wernicke was made, giving replacement treatment with thiamine and achieving dramatic improvement. Emphasizing the rarity of presentation of this entity is made, classically related to chronic alcoholism, as well as the importance of early diagnosis and proper treatment to prevent evolution to Korsakoff's psychosis and even death.Se reporta el caso de una gestante de 10 semanas hospitalizada por hiperemesis gravídica recibiendo tratamiento de reposición con líquidos y electrolitos, asi como piridoxina, gravol, cimetidina y diazepam. La paciente evolucionó con somnolencia, agitación sicomotriz, desorientación, nistagmus, ictericia, ataxia e hipoestesia a predominio de miembros inferiores. Se hizo el diagnóstico de encefalopatís de Wernicke, dándose tratamiento de reposición con tiamina y logrando espectacular mejoría.
