| Sumario: | [EN]Introduction: Fractures of the temporal bone may be associated with paralysis of the facial nerve, a complication that requires specialized intervention. Traditionally, visualization is done with a microscope or endoscope; however, the 3D exoscope is an emerging technology in otologic surgery. Objective: To present the first documented case of facial nerve decompression after temporal bone fracture using a 3D exoscope as a visualization system. Case description: A 70-year-old woman with delayed left facial paralysis after longitudinal fracture of the temporal bone. In view of the failure of conservative treatment and evidence of neural degeneration greater than 90 % in electroneurography, surgical decompression of the third portion of the nerve was performed using a transmastoid approach and the use of a 3D exoscope. The surgery allowed the preservation of hearing and the integrity of the ossicular chain. The patient experienced progressive functional recovery, reaching full facial mobility at two months postoperatively. Conclusions: The 3D exoscope showed excellent image quality and ergonomics, facilitating the precision of facial nerve decompression. This technique represents a promising alternative to improve outcomes in cases of facial paralysis secondary to temporal bone fracture. |
