Sumario:[EN] Introduction and objective: Otosclerosis is a bone disease of the labyrinthine capsule that presents clinically with generally transmissive hearing loss. In this case the treatment is to remove the stapes, attached to the oval window, and replace it with a prosthesis. Stapedial muscle tendon is usually sectioned. Material and Methods: We show anatomical evidence of stapes vascularization and evolution of the resonance frequencies in patients undergoing stapedectomy to which it is sectioned or preserves the stapedial tendon. Results: The results obtained in patients undergoing surgery of otosclerosis -stapedectomy without severing the tendon estapedial- show biological results (preservation of the vascular supply of the long process of the incus) and acousto-mechanical properties (preservation of the resonance frequency close to the patients otologically healthy) than they are superior to the others which the tendon are sectioned. Conclusions.- Current theoretical knowledge, the literature published on the subject and the results obtained by our group, make us consider the conservation of the stapedial tendon provides the patient approximately at least equal postoperative tone thresholds when stapedial muscle tendon is severed, with the the biological and acousto-mechanical advantages already exposed.
