In order to solve this problem and to improve the adherence in the chronically polymedicated population, the Personalised Medication Dosage systems become essential, so that the patient can take his/her medication, organized in weekly multidose blisters, at the community pharmacy. The project consists of several stages: first, a study of the proportion, causes and factors which determines the lack of adherence among the population in El Encinar is carried out through the design of a survey. This survey is also used to select the most appropriate subjects to receive the medication in PMDS devices, which makes it possible to implement the service at the pharmacy for 4 months. Throughout the procedure, all the relevant information is collected to finally determine and analyze the benefits that patients, pharmacists, pharmacies and health services obtain thanks to the PMDS, concluding that it is an useful tool to upscale adherence to treatments.[ES]La falta de adherencia al tratamiento farmacológico se presenta en los últimos años como una cuestión prioritaria de la salud pública a nivel mundial debido a sus nefastas consecuencias tales como incremento de fracasos terapéuticos y de costes sanitarios asociados. Como medio para corregir el problema y mejorar la adherencia de la población polimedicada crónica aparecen los Sistemas Personalizados de Dosificación (SPD), de forma que el paciente pueda recoger en la OF su medicación, organizada convenientemente en blísteres multidosis semanales. El proyecto realizado se divide en varias etapas: en primer lugar, se realiza un estudio de la proporción, causas y factores determinantes de la falta de adherencia entre la población de El Encinar mediante el diseño de una encuesta. Dicha encuesta sirve a su vez para la elección de los sujetos idóneos para recibir la medicación en dispositivos SPD, lo que permite implantar el servicio en la OF durante un total de 4 meses. A lo largo del procedimiento se recoge la información necesaria para finalmente determinar y analizar los beneficios que pacientes, farmacéuticos, farmacias y servicios sanitarios obtienen gracias al SPD, concluyendo que efectivamente constituye una útil herramienta para facilitar la adherencia a los tratamientos.Ediciones Universidad de Salamanca (España)20182018info:eu-repo/semantics/articleapplication/pdfhttp://hdl.handle.net/10366/137747reponame:GREDOS.
