| Sumario: | The current research work starts with the necessity of diagnosing the causes for the non-fulfillment of the treatment for the high blood pressure – HBP. The population involved was 30 patients and 20 medical doctors from the Manuel Ygnacio Monteros Hospital of the Social Security Institute of the City of Loja. A mixed paradigm was used – qualitative/quantitative, assuming the realization of a descriptive and transversal study. Several instruments were used in order to diagnose the causes for the lack of fulfillment, among them two tests to collect the medical criteria, general information, Batalla’s tests, Morisky’s, disease attitude test for patients. The obtained results demonstrate there is insufficient knowledge about the disease on the patient’s side, as well as there is little continuation, control, and explanation about the adherence to the treatment on the doctor’s side towards the patient. It was evident that there is an inadequate risk perception which represents the therapeutic non-fulfillment regarding the complications of HBP. It was proved that the little knowledge about the disease, the inadequate attitude towards the disease, and the presence of an unfavorable vision of this suffering interfere with the patients’ correct therapeutic adherence. It was concluded that there is a necessity to optimize the multidisciplinary and psych education work. There are affectations in the self evaluation of the hyper tense patients setting off insecurity and lack of partial or global efficacy towards the demand of the health situation which contributes to the failure in the therapeutic fulfillment. Which is why we could recommend that the health personnel’s assistance work addressed to the patient must be from a systemic perspective, in which care is relevant, health education, continuation and control of the cases, because of this, we propose as an essential part of this work a proposal in order to improve and increase the therapeutic adherence for the benefit of the patient, his/her family, and the hospital itself.
