Tradicionalmente, se ha creído que la alteración de la dimensión vertical oclusal (DVO) es un procedimiento dental que puede causar problemas como dolor muscular, desarrollo de signos y síntomas de trastornos temporomandibulares, dolor de cabeza y bruxismo, por lo cual es necesario garantizar que los tratamientos de ortodoncia sean seguros para la salud de la articulación temporomandibular (ATM). Hasta el momento no existe evidencia solida en la literatura reciente que evalué la asociación del aumento de la dimensión vertical oclusal (DVO) con fines ortodónticos y sus efectos en la ATM. Objetivo: Reunir evidencia científica donde se encuentren cambios en la posición condilar con el uso de aparatología de ortodoncia que aumente la dimensión vertical oclusal. Metodología: Se realizó una búsqueda sistemática en las bases de datos Medline (Pubmed), Scopus, Science Direct y BVS con los siguientes términos de búsqueda “Temporomandibular Joint", "Vertical Dimension", "Dental Occlusion" y "Orthodontics". Se consultaron las bibliografías de todos los artículos seleccionados para publicaciones adicionales y se realizó una búsqueda Manual en las diferentes bases de datos. Resultados: Tras realizar la búsqueda se en las diferentes bases de datos se encontraron 3667 artículos, de los cuales se seleccionaron 9 artículos para su análisis. Se encontró que la ayuda diagnostica más utilizada para la evaluación de la posición condilar es la tomografía computarizada de haz de cono. No se encontraron cambios estadísticamente significativos para; la posición condilar en sentido anteroposterior y vertical, la relación céntrica y la oclusión céntrica, ni en el volumen condilar. En cuanto a la posición condilar en el espacio articular solo dos artículos correspondientes a reportes de casos, reportaron cambios estadísticamente significativos, al igual que el índice del espacio articular. Conclusiones: Los conocimientos acerca de los efectos del aumento de la dimensión vertical oclusal sobre la ATM se han basado principalmente en reporte de casos y en estudio observacionales descriptivos más que en ensayos clínicos bien controlados. No se tiene evidencia suficiente y contundente sobre los posibles efectos del aumento de la DVO en la ATM. La evidencia disponible hasta el momento es débil y parece indicar que la ATM tiene la capacidad de adaptarse rápidamente a cambios moderados en la DVO, por lo tanto, no hay indicios de que una alteración permanente en el DVO produzca cambios significativos en la ATM y la posición condilar. Sin embargo, se necesitan estudios adicionales. |

Comportamiento De La Posición Del Cóndilo Mandibular Con El Aumento De La Dimensión Vertical Oclusal En Pacientes Tratados Con OrtodonciaOjeda Muñoz, Carlos AugustoUnigarro Marcillo, Marcelo AlejandroArticulación TemporomandibularDimensión Vertical OclusalOrtodonciaTG 2021 EOF 41172Temporomandibular jointOcclusal Vertical DimensionOrthodonticsTradicionalmente, se ha creído que la alteración de la dimensión vertical oclusal (DVO) es un procedimiento dental que puede causar problemas como dolor muscular, desarrollo de signos y síntomas de trastornos temporomandibulares, dolor de cabeza y bruxismo, por lo cual es necesario garantizar que los tratamientos de ortodoncia sean seguros para la salud de la articulación temporomandibular (ATM). Hasta el momento no existe evidencia solida en la literatura reciente que evalué la asociación del aumento de la dimensión vertical oclusal (DVO) con fines ortodónticos y sus efectos en la ATM. Objetivo: Reunir evidencia científica donde se encuentren cambios en la posición condilar con el uso de aparatología de ortodoncia que aumente la dimensión vertical oclusal. Metodología: Se realizó una búsqueda sistemática en las bases de datos Medline (Pubmed), Scopus, Science Direct y BVS con los siguientes términos de búsqueda “Temporomandibular Joint", "Vertical Dimension", "Dental Occlusion" y "Orthodontics". Se consultaron las bibliografías de todos los artículos seleccionados para publicaciones adicionales y se realizó una búsqueda Manual en las diferentes bases de datos. Resultados: Tras realizar la búsqueda se en las diferentes bases de datos se encontraron 3667 artículos, de los cuales se seleccionaron 9 artículos para su análisis. Se encontró que la ayuda diagnostica más utilizada para la evaluación de la posición condilar es la tomografía computarizada de haz de cono. No se encontraron cambios estadísticamente significativos para; la posición condilar en sentido anteroposterior y vertical, la relación céntrica y la oclusión céntrica, ni en el volumen condilar. En cuanto a la posición condilar en el espacio articular solo dos artículos correspondientes a reportes de casos, reportaron cambios estadísticamente significativos, al igual que el índice del espacio articular. Conclusiones: Los conocimientos acerca de los efectos del aumento de la dimensión vertical oclusal sobre la ATM se han basado principalmente en reporte de casos y en estudio observacionales descriptivos más que en ensayos clínicos bien controlados. No se tiene evidencia suficiente y contundente sobre los posibles efectos del aumento de la DVO en la ATM. La evidencia disponible hasta el momento es débil y parece indicar que la ATM tiene la capacidad de adaptarse rápidamente a cambios moderados en la DVO, por lo tanto, no hay indicios de que una alteración permanente en el DVO produzca cambios significativos en la ATM y la posición condilar. Sin embargo, se necesitan estudios adicionales.Associating the procedure of alteration in the occlusal vertical dimension (OVD), to problems of muscle pain, symptoms of temporomandibular disorders, headache and bruxism, the behavior of the temporomandibular joint (TMJ) is evaluated, with an increase in OVD in patients with orthodontics, a Scoping Review is performed, with Medline (Pubmed), Scopus, Science Direct and BVS databases; an also search terms: "Temporomandibular Joint". Bibliography research: selected articles, additional publications and specific search; from 3667 articles, 41 were preselected and 9 were analyzed. Scientific evidence on dependent condylar position changes, use of lifting techniques leading to augmentation, and answering the question about TMJ behavior, with increasing occlusal vertical dimension is searched. Most used diagnostic aid for evaluation of condylar position consisted of: cone beam computed tomography. No statistically significant changes were found in anteroposterior and vertical condylar position, relationship and centric occlusion, or in condylar volume. Two articles, case reports record statistically significant changes: condylar position in joint space, and index. Reported effects of the increase OVD on TJM, are mainly based on case reports and descriptive observational studies, overcoming controlled clinical trials. There is insufficient evidence on the effects of increased OVD on TMJ. With weak evidence available, indicating rapid TMJ adaptation capacity to moderate changes in OVD. TMJ changes and condylar position are not indicated due to permanent alteration of OVD; further studies are required.Introducción. -- 1.
