Sumario:Metamizole, commonly called dipyrone, is a drug widely used to relieve pain and reduce fever in various parts of the world, and is especially used in clinical situations involving these symptoms. Despite being effective, its use generates debate because of the risk of adverse reactions, including agranulocytosis, which is a rare condition, but which can be fatal. This work aims to analyze mainly the risks associated with undesirable adverse of metamizole, investigating its safety and the factors that may contribute to these events.  The research was carried out based on a literature analysis in the PubMed, SciELO and Virtual Health Library (VHL) databases. The descriptors used for the search were "metamizole", "dipyrone", "adverse effects", "agranulocytosis", "drug safety" and "pharmacovigilance", in Portuguese, English and Spanish. The inclusion criteria included articles published in the last 20 years, available in full text, that addressed the safety of metamizole, its adverse reactions, or pharmacovigilance studies involving this drug. Clinical studies, systematic reviews, were prioritized. The exclusion criteria included publications that did not have direct relevance to the topic, studies with incomplete or duplicate data, and research carried out on animals or that did not specify human populations.The research discusses the clinical circumstances in which the drug is recommended, the main dangers involved, and the correla-tion between dosage and response. The findings underscore the need to assess the safety profile of metamizole, especially in more susceptible groups, such as the elderly and individuals with comorbidities. The importance of this evaluation lies in the extensive use of metamizole, especially in nations where it presents itself as an affordable alternative for the treatment of pain and fever. This research aims to improve clinical practice in a safe way, offering recent data to health professionals and emphasizing the urgency of a conscious use of the drug.
