The classic manifestation of septic arthritis includes severe pain, even at rest, edema, increased temperature on site and reduced mobility of the joint. Microorganisms trigger an inflammatory response in the joint that creates the conditions for the recruitment of cells such neutrophils that are granulocytes specialized in the control of microorganisms. Receptors for the Fc portion of immunoglobulin play an important role triggering pro and anti-inflammatory cascades with the release of many cytokines, in order to control the disease and the inflammation. This study aimed to evaluate the role of FcγRIIb and FcγRIII in the bacterial clearance and the inflammatory response in the experimental model of septic arthritis. To this end, mice deficient for these receptors were infected with S. aureus and compared to wild type animals at times determined by preliminary experiments. FcγRIIb-/- animals had an anticipated leukocyte accumulation peak and a reduction of the bacterial load in the joint cavity, associated with increased neutrophil apoptosis and CXCR2 expression. Furthermore, FcγRIIb-/- animals had a greater amount of IL-1 cytokine in the joint on the first day of infection in relation to wild animals. Similar data were obtained on culture of macrophages stimulated with S. aureus. Such increased joint inflammation in the FcγRIIb-/- animals at the beginning of the response may be due to increased signaling by stimulatory FcγR receptors, since there is an increase in the Syk molecule in FcγRIIb deficient macrophages. On the other hand, the animals FcγRIII-/- showed an increase in leukocyte accumulation and bacterial load at later times of infection associated with increased expression of PD1, an exhaust clonal marker. It is concluded, therefore, that FcγR receptors are very important in S. aureus induced septic arthritis, and the absence of the inhibitory receptor FcγRIIb potentiates the inflammatory response in the first moments of infection, facilitating a better elimination of the bacteria. This response may occur as a function of the FcγRIII stimulatory receptor, since its absence impairs the bacterial joint control, maintaining an intense and persistent inflammation, possibly altering the functions of specific lymphocytes.

Diversos vírus, fungos e bactérias podem causar tal doença, mas Staphylococcus aureus é o principal causador, devido aos seus inúmeros fatores de virulência e de evasão do sistema imune. A manifestação clássica de artrite séptica inclui dor intensa, mesmo em repouso, edema, aumento da temperatura no local e redução da mobilidade da articulação. A presença de microrganismos desencadeia uma resposta inflamatória na articulação que cria condições para a chegada de células como os neutrófilos, importantes para o controle de microrganismos. Os receptores da porção Fc de anticorpos desempenham papel importante em doenças infecciosas, desencadeando cascatas pró e anti-inflamatórias, com liberação de diversas citocinas, auxiliando no controle da doença e da inflamação gerada. O objetivo deste trabalho foi avaliar o papel dos receptores FcγRIIb e FcγRIII no controle bacteriano e na resposta inflamatória articular após a infecção por Staphylococcus aureus em camundongos. Para isto, animais deficientes para estes receptores foram infectados com S. aureus e a resposta inflamatória desencadeada foi comparada à de animais selvagens. Os animais FcγRIIb-/- apresentaram pico de acúmulo de leucócitos antecipado e redução da carga bacteriana na cavidade articular, associado a um aumento de apoptose de neutrófilos e maior expressão de CXCR2. Ainda, animais FcγRIIb-/- tem uma maior quantidade da citocina IL-1 na articulação já no primeiro dia de infecção em relação aos animais selvagens. Dados similares foram obtidos em cultura de macrófagos estimulados com S. aureus. Essa maior inflamação articular nos animais FcγRIIb-/- no início da resposta pode ser devido à uma maior sinalização por receptores FcγR estimulatórios, uma vez que há um aumento da molécula Syk em macrófagos deficientes para FcγRIIb. Por outro lado, os animais FcγRIII-/- apresentaram aumento no acúmulo de leucócitos e de carga bacteriana em tempos mais tardios de infecção, associado ao aumento da expressão de PD1 em linfócitos, um marcador de exaustão clonal. Conclui-se, portanto, que os receptores FcγR participam ativamente da inflamação articular causada por S. aureus, sendo que a ausência do FcγRIIb, receptor inibitório, potencializa a resposta inflamatória nos primeiros momentos de infecção, facilitando uma melhor eliminação da bactéria.
