The health professionals approached the initial difficulty of implementation due to resistance by some and insecurity due to lack of training, however, after proper training and correct knowledge of how the disclosure works, its implementation was properly implemented within the patient safety process, becoming routine practice when facing moderate, severe and fatal adverse events. With this, it is possible to observe its relevance within patient safety, as it promotes transparency among professionals, patients and families, brings improvements in the quality of health care, encourages notification by professionals, as it addresses the just accountability culture.Este estudo objetiva compreender qual o significado do disclosure para o paciente e seus familiares após recebê-lo e o que envolve seu estabelecimento no ambiente hospitalar. Estudo de abordagem qualitativa, descritiva e exploratória. A população foi constituída por pacientes e seus familiares – receberam o disclosure – e por profissionais da saúde (enfermeiros e médico) – realizaram o disclosure –, em um período de dois anos antes da coleta dos dados. Foi aplicado um questionário semiestruturado e com questões abertas. Os resultados evidenciam que é importante, pois demonstra a transparência do hospital com os mesmos e a busca por uma melhor qualidade na assistência. Para os profissionais da saúde, no início da realização do disclosure houve resistência por alguns, além da insegurança devido à falta de treinamento. Sinalizam que após o treinamento e conhecimento correto do funcionamento do disclosure, sua execução ocorreu adequadamente dentro do processo de segurança do paciente, tornando-se prática rotineira ante a eventos adversos moderados, graves e fatais. Para concluir, pode-se observar a relevância do disclosure, pois promove a transparência entre profissionais, pacientes e familiares, traz melhorias na qualidade da assistência em saúde e incentiva a notificação por parte dos profissionais, pois aborda a cultura justa de responsabilização.
